The new default, an exceptionCount of 0 — which is the wipe, observable — and wipedExceptions, how many per-tool decisions it destroyed. A confirm dialog should warn with the count read from _lookup BEFORE the flip and echo this afterwards; the two can differ if somebody else was editing.
